New Patients

We appreciate you choosing our practice. Pediatric and Adolescent Medicine would like to offer a warm welcome to you and your family. We realize that you have many choices when it comes to pediatricians in the area and we are honored that you have chosen our physicians to provide your family the best pediatric medical services possible. We work hard every day to maintain the most up to date medical care available. We accept most insurances, but if you are unsure if we accept your policy, please feel free to call our office at 804-358-2361 to verify our acceptance of your particular insurance policy.


Below are some items that will be needed at your visit:

  • Bring in all insurance cards
  • Have the co-pay if your insurance requires a co-pay
  • Provide a picture i.d.
  • If transferring from another doctor, bring a copy of the vaccination records and have the patient’s records forwarded to our office.
  • To make the registration quicker, go to the practice website and fill out new patient forms on line before the appointment.
  • As a new patient, come in 30 minutes before your appointment time to complete all forms if you don’t take advantage of the on-line forms. If you do complete the forms on-line, come in 15 minutes early.
  • If you have a newborn, ask the hospital to fax over the newborn records to 804-359-0949.

What to expect

At your first visit, your doctor will do a complete review of your child’s medical history and develop a plan to address the on-going health of your child. Having your child’s records or newborn information will make this process flow much faster. Additionally, the doctor will discuss the vaccination record and setup a schedule for any vaccinations that may not be up to date.
